Update switch:
- simplify the code
- make silent the invisible switches
- fix "switch inside switch" bug(i)
- fix "in/out" switch bug(ii)
- enable inside highlighting(iii)
(i) if a switch A is reached while the ball is inside an other switch B, then A is not activated.
(ii) the ball must now be completely out in order to reactivate a switch.
(iii) the halo is stronger when a ball is inside a switch.

Fix high-score saving.
Load hs only when needed.
Activate hs level loading.
Try a different display of sets.

Another strong code change. This one focusses Set management:
* move many set functions from levels.[hc] to set.[ch]
* create structures : score, set and et_level
* rewrite high score management
* add "collect coins and reach the goal" high score
* put challenge mode button under "normal" and "practice"
